CLEMSON PILATES SOCIAL LIABILITY WAIVER AND CANCELLATION POLICY

Client Release Form

1. Pilates Social Clemson is not liable for any injury, physical or emotional, sustained while on the premises of 101 McCollum Street, Clemson, SC, 29631.
2. Client must inform instructor prior to lesson of any physical, mental, or medical changes.
3. 12-hour notice is required for cancellation, or full payment is required.
4. There is no refund for any unused credits each month.
5. A NO SHOW FEE of $15 applies if 12-hour notice was not received, in addition to monthly fees. NO EXCEPTIONS FOR ANY REASON. This will be strictly enforced.
6. Client must arrive on time for their class. If the client is 10 or more minutes late, the credit will be forfeited and paid in full.
7. Pilates Social Clemson, LLC is not responsible for lost or stolen items.
8. It is the client's responsibility to know his or her credit balance.
9. Client is to be respectful of other clients coming and going and respect the other client's time and lesson by arriving and departing quietly.
10. Failure to comply to any of these requests is an automatic forfeit of a class credit.
11. Instructor reserves the right to end the lesson at any time if it is in the best interest of the client or other clients in the class.
12. Client may be removed from the studio by the instructor at any time.
13. Client shall not consume alcohol or drugs prior or during session. Instructor will be informed of any prescription drugs taken prior to the lesson and the instructor reserves the right to end the lesson.
14. All students must wear grip socks to class. NO EXCEPTIONS CAN BE MADE. If you forget a pair of grips socks, they will be available for purchase at the studio (while supplies last).
15. All students must wipe down their equipment after class with the sanitary wipes provided by Pilates Social Clemson, LLC.
